✔️ PSA Birth Certificate (for Grades 7, 11, transferees, Balik-Aral, and SNED learners)
✔️ 2 pcs. 2x2 picture with red background (must be accompanied by parent/guardian)

📌 Enrollment Procedures:
🔹 Step 1: Brigada Eskwela
Report to your respective Class Adviser and classroom for the conduct of BE.
🔹 Step 2: Enrollment
Secure and accomplish the enrollment form and submit the required documents to the adviser. Wait for the confirmation.

📣 Important Reminder:
Transferees, Balik-Aral, OHSP, and SNED learners are advised to proceed to the Guidance Office for enrollment confirmation.
