Tungod sa pandemya, nanginahanglan an Surigao del Norte National High School nan ato pagtabang-tabang para mapadajun sanan maplastar ang pagsugod nan klase. Sa uman na pamaagi kaw makatabang?

Ang ato skuylahan nanginahanglan nan mga:

1. Learner’s kit - notebooks, intermediate papers, ballpens, and etc.
2. Learning gadgets – laptop, tablet, or cellphone
3. Medical devices and equipment – infrared and digital thermometer
4. Health and sanitation supplies – face masks, face shields, disinfectants, and vitamins
5. Module supplies - Risograph machine, Riso ink, reams of A4 and long bond paper, printer and printer ink
6. Construction supplies - paint, nails, plyboards, and etc
7. Gardening tools and supplies – seedlings, pots, fertilizers, trowel, bolo knife, and etc.

ENROLLMENT REQUIREMENTS:

for Grade 7, Balik-Aral & Transferees
✔️ Original copy of Report Card
✔️ Original copy of Good Moral Certificate
✔️ PSA Birth Certificate (photocopy)
✔️ NCAE Result (Gr. 11 only)
✔️ ALS Certificate of rating

for Grade 8, 9, 10 & 12
✔️ filled out Learners Enrollment & Survey Form (LESF)

*Old students are pre-enrolled to the next grade level by the previous advisers. They will contact you otherwise, call or text them for inquiries.

ENROLLMENT PROCEDURE

1. Click the link below according to your appropriate level and fill out the Learners Enrollment and Survey Form (LESF).
2. You will receive an email/text as confirmation of your submission.
3. For incomplete or submitted documents with issues, you will receive an email/text with instructions on how to complete it.

For Physical enrollment:

1. For those who cannot enroll online, printed LESF will be available at the enrollment booth near the guard house/entrance gate (gate 1) .
2. Please bring your own pen and make sure to return the filled out LESF on the box provided and wait for a text confirmation.
3. Learners who were able to enroll online do not need to fill out the printed form to avoid duplication. 🎗
4. The school strictly implements a “No-Mask No-Entry Policy” and physical distancing measures. Foot baths and hand-washing stations are available at the entrance gate.
5. Only PRE-SCHEDULED TRANSACTION will be accommodated.
