Good day, AFONHS pips! Enrollment is just around the corner. Here are the steps you need to follow after getting an "Enrollment Slip";

Step 1 - Undergo "Reading Assessment"
Step 2 - Check your Body Mass Index
Step 3 - Participate in the clean-up activity at the school
Step 4 - Submit the necessary documents to be officially enrolled.
Note: Ensure that the teacher-in-charge for each step has clearly attached his/her name and signature.

- Accomplished Basic Enrollment Form
- SF 9 (Report Card) with LDA
