Must be a Filipino citizen
K–12 Graduate, ALS Graduate, or Old Basic Education Curriculum Graduate
Not currently in training
Not employed
Willing and committed to study continuously for 3 years

Requirements:
Original SHS Report Card (Form 138 / Form 137)
Original Certificate of Good Moral Character
Photocopy of PSA Birth Certificate
Photocopy of Marriage Certificate (for married applicants)
Three (3) colored ID pictures (white background):
