All NEW AND ONGOING CHED GRANTEES are requested to submit a scanned copy or screenshot of ANY ONE (1) of the following:

a. Front page of the LANDBANK Card
b. LANDBANK Deposit Slip
c. For grantees with online banking only: A screenshot of the LANDBANK Mobile Banking Home Page showing the Account Number

📌 Important:Please ensure that the submitted copy or screenshot is clear and readable and that the Account Number is visible for verification and documentation purposes.

📄 Submission Format: The document must be printed on LONG BOND PAPER.

📢 ATTENTION, ALL CHED GRANTEES (AY 2026–2027)

All official CHED grantees are required to report to the Scholarship Office to complete the CHED Grantee Information Survey through a personal interview.

The information gathered is necessary for official scholarship monitoring, documentation, and other CHED-related purposes. All information provided will be treated with strict confidentiality.

MWF (1:30 PM-3:30 PM)
