Landmark: We are directly across from Supreme Summit Motors, Inc., and just a 4-minute walk from SM City Sto. Tomas and S&R (coming from the Rotonda direction).

📄 REQUIRED DOCUMENTS
Please ensure you bring the following complete requirements:
ID Pictures: Three (3) pieces of 2x2 ID picture (white background with name tag in the format: Last Name, First Name, Middle Name)
Birth Certificate: Three (3) photocopies of your PSA Birth Certificate
Good Moral: Certificate of Good Moral Character
Report Card: Original Form 138 (Grade 12 Report Card) and two (2) photocopies
Folders: Three (3) pieces of long colored folders with fasteners.
