SLSU-San Juan School Clinic

The SLSU-San Juan School Clinic is conducting its monthly tooth extraction service. This initiative provides essential dental care to students and employees, ensuring their oral health is maintained.

📢 attention, students and staff! Dengue is on the rise in our municipality and it is important that we do our part to stay safe. Here are a few steps to protect urself and our school. Let us practice the 4'S kontra dengue:
1. Search & destroy the breeding places of mosquitos. Keep our environment clean
2. Self Protection measures by using insect repellent, using mosquito nets, avoid wearing dark clothing, wear long sleeves and pants
3. Seek early consultation. If u have fever for 3 days, body weakness, muscle pain & nausea & vomiting, don't hesitate to go to the clinic for consultation.
4. Say "yes" to fogging when an outbreak occur.

Together, we can make our school safe and dengue-free.

To all freshmen who are about to enroll in SLSU-San Juan, here are the medical documents that you need to bring during laboratory examinations and the issuance of medical certificate.

Step 1. print the following attached documents. (no need to visit the school clinic to get such documents, for easy access and saves your time and money)
Note:
*Medical Certificate and Patient's Health Record should be printed in A4 size paper.
* Laboratory Request can be printed 1/4 size of short size bond paper.

Step 2. If you have all the documents, you can proceed to the hospital/clinic of your choice for the laboratories and issuance of medical certificate.
Note: only the LABORATORY REQUEST and MEDICAL CERTICATE FORM is needed in this step.

Step 3. Please fill out the PATIENT'S HEALTH RECORD FORM and let your parent or guardian sign.

Step 4. Submit all the forms in the school clinic together with the other school requirements.
