✅ Photocopy of Birth Certificate and Valid ID
✅ High School Diploma or Form 137
✅ Latest passport-size ID photo and 1×1 photo (2 copies each)
✅ To qualify, the combined annual household income must not exceed ₱300,000.

Submit either of the following:
• Certificate of Indigency (from Barangay)
• BIR Form 2316 (if parent/guardian is employed)
• Latest Income Tax Return (ITR) of parent/guardian
