Kindergarten
• Photocopy of PSA Birth Certificate
• Photocopy of Baptismal Certificate
• Must be 5 years old by December 31, 2026
• Day Care Completion Certificate or Report Card
• Two (2) copies of a 2x2 photo (for new learners)
Incoming Grade 1 & Transferees
• Photocopy of PSA Birth Certificate
• Photocopy of Baptismal Certificate
• Kindergarten Completion Certificate
• Report Card / ECCD Assessment Checklist
• Certificate of Good Moral Character
Grades 7–10
• Photocopy of PSA Birth Certificate
• Photocopy of Baptismal Certificate
• Confirmation Certificate
• Report Card
• Certificate of Good Moral Character
• For incoming Grade 7: Proof of Parent/Guardian’s Income (ITR, Certificate of Employment, or Certificate of Indigency)
Please bring your child with you for ID picture taking.
Incoming Grades 4 and 7 will also take their assessment test on the same day.
