* Duly accomplished Personal Data Sheet usung CS Form No. 212, revised 2025.
*Copy of any proof of LET Eligibility (license or rating)
* Photocopy of Transcript of Records (TOR)
* Certification from current school stating enrollment in an Education course (for pre-service teachers only)
*Original copy of certification of good moral character issued by the authorized entity for pre-service teachers

NOTE: *DepEd teachers and DepEd teacher-applicants shall submit Annex A only. Applicants who are not DepEd teachers nor DepEd teacher-applicants shall submit the above-mentioned documents, whichever is applicable.
