All clinician participants are required to accomplish and submit their Parent’s Consent and Waiver Form through the provided Google Drive link.

Please be informed that only those with approved and properly submitted waivers will be eligible to claim their tickets. Failure to comply will result in denial of access to the event.

Clinicians, the time has come to capture your beautiful smiles! As we prepare for the upcoming pictorial, please make sure to check your assigned schedule as we updated the list to give way for those who have classes and please be guided by the reminders provided to ensure a smooth and organized session.

We advise all clinicians to come in complete uniform, arrive on time, and bring their official receipt for verification upon arrival. A holding area will also be provided; however, everyone is highly encouraged to come fully prepared to help maintain an efficient flow of the pictorial.
