I saw this one online... Self evaluation sheet, I made our own, in place of the stars I chose happy faces. I also added remarks, and had to write on print so she can read it. We'll be doing this every friday (end of the week). The first page consists of the weekly planner (if she needs to set a deadline in the art projects she's doing or an on line work that she needs to continue or submit or print out), then there's the academic subjects that she has and the topics that were tackled per day and grades if ever there are activities she answered.
I loved this evaluation (back part) area where she is able to see and evaluate how she did for the week. I also use this part so she can reflect on the actions she did such as respecting others and authorities, the creations of God and so on. I remembered working as a QA there was an EPR where you rate yourself and submit it to your supervisor. She can also see how I evaluate her performance as a whole not only academically but how she interrelate to other people.
