Please be informed that the application for graduation is scheduled to start this week until the last week of May. Kindly secure your application form from your program head/coordinator and prepare the requirements listed below. Thank you!
Note! Requirements should be photocopied in color on long bond paper
APPLICATION FOR GRADUATION REQUIREMENTS
1. Application Form
2. OJT Certificates
3. PSA Marriage Contract (if Married)
4. PSA Birth Certificate
5. Prospectus with Grades
6. Evaluation of Grades from 1st yr to 4th yr
7. Certificate of Registration from 1st yr to latest
• including those COR from 2011 to present for Returnee or Irregular Students
8. Special Order Receipt (SO Payment)
9. Long Colored Folder (per department)
• Red for Criminology
• Blue for Education
• Yellow for BSBA
• Violet for Tourism
• Green for BSAB
• Orange for Entrep
10. Binder Clip(to hold your Requirments)
