NOTE: Please prepare the following prior to the date of enrollment.
Grade 7 Requirements
1. PSA Certified Birth Certificate
2. Grade 6 Report Card or equivalent for Elementary completion (ALS/PEPT certificate)
3. Recent 2×2 ID Photos
4. Proof of Financial Need — one of the following:
• Latest Income Tax Return (ITR)
• Certificate of Tax Exemption (if not required to file ITR)
• Municipal Certificate of Unemployment of parent/guardian
• Certificate of Indigency/Proof of Income
ESC Slots for incoming Grade 7 students are on a FIRST COME, FIRST SERVE basis.
Grade 11 Requirements
Public School Students (Automatically Qualified Voucher Beneficiary):
1. Grade 10 Final Report Card with Learner Reference Number (LRN)
2. PSA Birth Certificate
ESC Grantees (Private JHS):
1. ESC Certification Letter from Junior High School (showing ESC grantee status)
2. Original Grade 10 Final Report Card
3. PSA Birth Certificate
Non‑ESC Private School Grade 10 Completers (Voucher Applicants):
1. Qualified Voucher Applicant (QVA) Certificate
2. Grade 10 Final Report Card with LRN
3. PSA Birth Certificate
