PEDRO VALENZUELA ELEMENTARY SCHOOL is NOW accepting TEACHER I Applicant starting May 15-31, 2023.
REMINDERS.!!!
🔸 All interested applicants shall prepare two (2) sets of pertinent documents, one (1) set of photocopies of documents to be verified against the original copy by the School Sub-Committee and one (1) set of original documents as basis of certification by the School Head. Only the set of verified photocopies of documents shall be submitted to the school.
🔸Applicants shall also accomplish the online application form to the link bit.ly//sdoneranking on or before May 31, 2023.
🔸 For the requirements.
The following pertinent document to be submitted shall be properly labeled with dog-ears/side tabbing sequenced and arranged by letters in GREEN folder:
A. Letter of intent addressed to the Schools Division Superintendent.
B. Duly Accomplished PDS (CS Form 212 , Revised 2017) with Work Experience Sheet, if applicable;
C. Photocopy of valid and updated PRC license / ID; if applicable
D. Photocopy of scholastic/ academic record such as but not limited
to Transcript of Record (TOR) and Diploma, including completion of Graduate and post-graduate units/degree , if applicable;
E. Photocopy of Certificate/s of Training; if applicable;
F. Photocopy of Training/s , if applicable;
G. Photocopy of Certificate/s of Employment , whichever is/are applicable;
H. Photocopy of Latest Appointment; if applicable; and
I. Checklist of Requirements and Omnibus Sworn Statement on the Certification on the Authority and Veracity (CAV) of the documents submitted and Data Privacy Consent Form (Enclosure) notarized by authorized offiicials.
