This advisory serves to announce the official schedule for ๐ˆ๐ƒ ๐ญ๐š๐ค๐ข๐ง๐  and ๐ˆ๐ƒ ๐ซ๐ž๐œ๐ž๐ข๐ฏ๐ข๐ง๐  for the period of Monday through Saturday. Please take note of the following operational guidelines before proceeding to the office.

The MIS Office operates on two daily sessions: the ๐ฆ๐จ๐ซ๐ง๐ข๐ง๐  ๐ฌ๐ž๐ฌ๐ฌ๐ข๐จ๐ง (๐Ÿ–:๐Ÿ‘๐ŸŽ ๐€๐Œ โ€“ ๐Ÿ๐Ÿ:๐Ÿ‘๐ŸŽ ๐€๐Œ) and the ๐š๐Ÿ๐ญ๐ž๐ซ๐ง๐จ๐จ๐ง ๐ฌ๐ž๐ฌ๐ฌ๐ข๐จ๐ง (๐Ÿ:๐ŸŽ๐ŸŽ ๐๐Œ โ€“ ๐Ÿ’:๐ŸŽ๐ŸŽ ๐๐Œ). The office observes a lunch break from ๐Ÿ๐Ÿ:๐Ÿ‘๐ŸŽ ๐€๐Œ ๐ญ๐จ ๐Ÿ:๐ŸŽ๐ŸŽ ๐๐Œ, during which no transactions will be processed. It is important to distinguish between the two services offered: ๐ˆ๐ƒ ๐‘๐„๐‹๐„๐€๐’๐ˆ๐๐† is available during ๐›๐จ๐ญ๐ก ๐ฆ๐จ๐ซ๐ง๐ข๐ง๐  ๐š๐ง๐ ๐š๐Ÿ๐ญ๐ž๐ซ๐ง๐จ๐จ๐ง sessions, whereas ๐ˆ๐ƒ ๐“๐€๐Š๐ˆ๐๐† is conducted during the ๐€๐…๐“๐„๐‘๐๐Ž๐Ž๐ ๐’๐„๐’๐’๐ˆ๐Ž๐ ๐Ž๐๐‹๐˜. Students must plan their visit accordingly based on the service they require.

This is the ID schedule for this first semester, A.Y. 2025-2026.
๐Ÿ‘‰๐Ÿป FIRST TIME ID: There is no charge for the first issuing of an identification card.
๐Ÿ‘‰๐Ÿป ID REPLACEMENT
To replace an old/worn-out ID, the student:
a. should surrender his/her ID at the DSA office and claim the official letter for the photographer
b. pays the RFID fee for โ‚ฑ210.00 with sling, โ‚ฑ150.00 w/o sling, โ‚ฑ70.00 just sling, and
c. presents the official receipt and official letter for the photographer to the official photographer of the school.
๐Ÿ‘‰๐Ÿป LOST ID
To replace a lost ID card, the student:
a. presents an affidavit of loss notarized by a lawyer to the Official Photographer,
b. pays the I.D. fee, and
c. presents the official receipt to the official photographer of the school.
