Requirements:
1. SF9
2. Certificate of Good Moral Character (G11)
3. Birth certificate (copy)
4. ID pictures (1x1)
5. Vaccination card (for vacc status)

For irregular students (balik-aral, repeaters)
Additional requirements:
Interview with parent or guardian (Guidance counselor)
Letter signed by both student and parent/guardian
