For those who are interested to avail the scholarship please visit OSPA TRAINING CENTER located at Carlota Hills, Can-adieng, Ormoc City and please bring the following requirements:
5 Photo Copies- PSA/NSO Birth Certificate
5 Photo copies-High School Card/Form137/High School Diploma/ ALS Card/ School Certification
1 Photocopy of Certificate of Indigency
5 pcs.- Passport Size Id Picture (white background, suit attire with nametag)
5 pcs. 1x1 ID picture
1 white long Folder
