March 1 - Sabin Resort Hotel

The amount of 600 pesos will be collected by College of Education officers as soon as possible. Deadline of payment will be on February 23, 2024.

Note: This payment is compulsory.

𝐄𝐋𝐈𝐆𝐈𝐁𝐈𝐋𝐈𝐓𝐘 𝐂𝐑𝐈𝐓𝐄𝐑𝐈𝐀:

1. A Filipino Citizen.

2. Applicant and his/her family are permanent residents of the City of Ormoc.

3. With good moral character and without any derogatory record.

4. A Grade 12 student with at least 80% average grade and no failing grade in any subject during his/her Grade 11 provided further that he/she finished his/her Grade 12 and graduated his/her senior high school graduate with at least 80% average grade and no failing grade in any subject during his/her Grade 12.

5. If currently enrolled in college, applicant must have a weighted average of at least 2.5 or 80% in the semester in college prior to the application to scholarship and with no failing grade and no incomplete grade in that semester prior to the application.

6. Has passed the entrance examination, if required, of the school where he or she intends to enroll.

7. From a poor family especially those classified as poorest of the poor.

8. Not presently enjoying any scholarship or study grant.

𝐑𝐄𝐐𝐔𝐈𝐑𝐄𝐌𝐄𝐍𝐓𝐒:

1. 1PC 2X2 ID Picture

2. Certificate of Good Moral Character from the School (Original)

3. Certificate of grade from the Registrar with an average of 2.50 or 80% with no failing grade (Original)

4. Latest BIR Income Tax Return (ITR) or certificate of tax exemption of applicant’s parents/guardians or Affidavit of applicant’s parents/guardians stating the reason for not filing the ITR (Original)

5. Certification of the Basic Salary of the applicant’s parent/s from HR (if parents are employed) (Original)

6. Barangay Clearance attesting to residency (Original)

7. Electric Bill and Water Bill for the past month (Photocopy)

8. Place all the requirements in a LONG BLUE FOLDER (not plastic)
