STEPS TO ENROLL AT ST. FRANCIS CATHOLIC SCHOOL OF BATAAN, INC. (Please follow them religiously)

FOR NEW PUPILS:
1. Fill in the application for Pre- elem and Grade school admission.
2. Submit the required documents (please put them all in a long brown envelope)
2.1 Form 138
2.2 Birth Certificate (photocopy only) preferably NSO certified
2.3 Baptismal Certificate (Certified true copy andphoto copy)
2.4 Marriage contract for parent (photocopy only)
2.5 2 copies of 2x2 and 2 copies of 1x1 pictures
2.6 schedule entrance test ELEMENTARY ONLY

3. Pay the registration/ miscellaneous fee as required payment upon enrollment

NOTE: Follow up also the other fees (notebooks/ diary, textbooks, school and P.E. uniforms)
---TEXTBOOKS MUST NOT BE GIVEN UNLESS YOU PAY THEM IN FULL. Then get your official receipt.

4. Sign the Parents/ Students conforme and submit to the office for confirmation of enrollment in St. Francis Catholic School of Bataan, Inc.

FOR OLD PUPILS:
1. Submit/ REturn the form 138 (Learners Report Card) to the office
2. Pay the registration/ Miscellaneous fee as required payment upon enrollment. Then get your official receipt.
3. Sign the Parents/ Students conforme and submit to the office for confirmation of enrollment in St. Francis Catholic School of Bataan, Inc.
