Triggers: Looking at another student's paper, attempting to communicate, possessing unauthorized materials, or frequent suspicious movement.
Action Required: The proctor will issue a direct verbal warning and log the student's name for close monitoring.

Triggers: Repeating a Level 1 offense, passing or receiving answers, using unauthorized notes/cheat sheets, using electronic devices, or copying answers.
Action Required: The proctor will immediately confiscate the unauthorized materials and document the incident. The student will be allowed to finish the exam under close surveillance. The incident will be formally reported to the subject teacher and the academic coordinator.

The case will be forwarded to the Academic Office for disciplinary action in alignment with school and DepEd policies. Any future special examination will require administrative approval.
