A. If SBCA MOA WITHOUT CHANGES FROM THE COMPANY-- print 4 copies , EDIT THE PART OF THE DEAN AND THE SCHOOL. DO NOT INCLUDE ALL IN THE MOA TEMPLATE ONLY RETAIN YOUR RESPECTIVE SCHOOL AND THE NAME OF YOUR DEAN. SUBMIT TO THE SIP OFFICE

B. If the MOA TO BE USED IS COMING FROM THE COMPANY-- YOU MUST EMAIL THE SOFTCOPY TO THE EMAIL OF THE SIP COORDINATOR:
