This survey is being conducted in support of ongoing discussions and policy considerations concerning the use of social media and digital devices. It also aims to gather relevant information that may serve as a basis for evidence-based discussions, recommendations, and possible policy measures regarding the use of digital devices and social media within school premises.
SURVEY DETAILS
Deadline: On or before September 18, 2026
For students below 18 years old, a Parent/Guardian Consent Form is provided in the final part of this memorandum. The form must be duly accomplished before proceeding with the survey.
REMINDERS
* All concerned individuals are encouraged to accomplish the appropriate survey, where applicable, and facilitate its dissemination to the intended respondents within their respective areas of responsibility.
* Participation is voluntary. No individual shall be compelled to participate, provide a particular response, or submit screenshots, copies, or individual responses as proof of participation.
* All survey-related activities shall strictly observe Republic Act No. 10173 (Data Privacy Act of 2012) and other applicable data privacy
and information security policies.
* Monitoring and reporting shall be limited to aggregate participation and dissemination data. Individual responses and personally identifiable information shall not be collected, disclosed, or reported beyond what is necessary and authorized for the conduct of the survey.
* All concerned are reminded to ensure that the survey is administered responsibly, ethically, and in accordance with existing school policies and data privacy principles.
