The National Library of the Philippines (NLP) has released 𝐑𝐞𝐯𝐢𝐬𝐢𝐨𝐧 𝟐 of the 𝗖𝗼𝗽𝘆𝗿𝗶𝗴𝗵𝘁 𝗥𝗲𝗴𝗶𝘀𝘁𝗿𝗮𝘁𝗶𝗼𝗻 and 𝗗𝗲𝗽𝗼𝘀𝗶𝘁 𝗔𝗽𝗽𝗹𝗶𝗰𝗮𝘁𝗶𝗼𝗻 𝗙𝗼𝗿𝗺. This updated version is now the official form required for all new applications.

Pursuant to Office Memorandum No. 115, s. 08-2026 issued by the Office of the URS President, office operations shall temporarily observe a work schedule from 8:00 a.m. to 5:00 p.m. on 17-20 August 2026 (Monday to Thursday) and office operations on 21 August 2026 (Friday) shall be suspended pursuant to Proclamation No. 1006, s. 2025 which was declared a Special (Non-Working) Day.
Regular Work schedule (7:00 a.m. to 6:00 p.m., Monday to Thursday) shall resume the following workweek, unless otherwise provided.
For information and proper guidance of all concerned.
