Fill out the Online Pre-Enrollment Form and provide complete and accurate information.
This form will also serve as your confirmation of intent to enroll at PCCM.

ADVISORY: SHIFTING OF PROGRAM
(for Old Students only)

All students who have applied for a change of program are requested to submit their duly accomplished Shifting Form on or before June 15, 2026.

Failure to submit the required form within the prescribed period shall be construed as withdrawal of your intent to shift programs.
