๐. Print all the downloaded forms on A4-sized paper and fill them out completely.
๐. Take the Laboratory Request form to any public or private hospital or clinic to have the required laboratory tests performed.
๐. After completing the tests, bring the results with you to your enrollment appointment and present them to the DEBESMSCAT Medical Officer to receive your medical certificate.
๐. Bring all completed forms: Individual Student Inventory (ISI) form, Studentโs Health Record, and Data Consent form on your enrollment date.

Claim your individual Result Slip, and submit your accomplished Individual Student Inventory Form.
๐ฆ๐๐ฒ๐ฝ ๐ฎ: COLLEGES
Proceed to the college where you have chosen your course.
- For the College of Engineering qualifiers, you must take a short qualifying test. (Please bring a scientific calculator.)
- For the College of Education and the BS Agriculture program qualifiers, you must attend a face-to-face pre-screening interview.
If you successfully pass and meet all admission criteria in your respective assessment, you will be offered a slot in the program and then issued a certificate of passing.
- For qualifiers in non-board programs, No Interview will be conducted; instead, you must request a certificate of acceptance from the department heads or the college of your chosen course before you enroll.
๐ฆ๐๐ฒ๐ฝ ๐ฏ: MEDICAL SERVICES
Submit your completed Studentsโ Health Record Form, your original chest x-ray (PA View), and drug test results. If you are applying for the BSIT Food Technology program, you must also submit an additional test result for Hepatitis B. The College Medical Officer will issue a Medical Certificate upon receipt of all required laboratory test results.

Submit all required enrolment documents for evaluation and verification. If everything is complete, the assigned staff will process your enrollment and provide you with an Enrollment and Assessment Form, which will be printed and stamped with โOfficially Enrolled.โ If you are not covered by RA 10931, proceed to the Cashierโs Office to pay any fees, then return to the Office of the Registrar for validation and to have the Enrollment and Assessment Form stamped with โOfficially Enrolled.โ

Sagot: Based sa unang post ng Office of Admission, Sa June 3, 2026 mag uumpisa ang interview sa College of Education at BS Agriculture program, at qualifying test sa College of Engineering, pero mas hinihikayat namin na magpa medical/laboratory test nalang muna at asikasuhin ang requirements sa enrolment bago pumunta dito sa DEBESMSCAT para hindi kayo pabalik-balik. Pwede pa naman magpainterview hanggang sa last week of July, kaya meron kayong two months.
