Please be reminded to strictly comply with the Schedule of Payment and Claiming of Test Permit/Admission Slip for the AY 2025–2026, 2nd semester,Final Examination.
📌 Reminders for Students:
• Please secure your Clearance Form on your assigned date for claiming your Clearance Slip at the Accounting Office. Kindly present your passbook for verification and/or updating of your account balance, if any. Ensure that your Clearance Form is duly signed by the Accounting Staff before proceeding to the Cashier’s Office for payment of your dues.
Note: Payment of dues may only be made once your Clearance Form has been properly signed by the Accounting Staff, Ms. Angeli L. Lepiten.
• Make sure that all required signatories have completed and signed your Clearance Form, following the designated order indicated on the form, prior to claiming your Test Permit/Admission Slip at the Accounting Office.
Failure to comply with the given instructions and schedule may result in delays or possible restrictions in taking the examination. Your full cooperation is highly expected.

All students are required to strictly follow the assigned schedule to ensure a smooth and orderly process:
• March 25, 2026 (Wednesday) – All First Year Students
• March 26, 2026 (Thursday) – All Second Year Students
• March 27, 2026 (Friday) – All Third Year Students
• March 28, 2026 (Saturday) – All Fourth Year Students

📌 Reminders for Students:
• Secure your Clearance Form one (1) week before your scheduled date of payment and claiming of your Test Permit. Present your passbook and Clearance Form to the Accounting Office for verification and/or updating. Ensure that your Clearance Form is signed by the Accounting Staff before paying your dues at the Cashier’s Office.
Note: You may pay your dues once your Clearance Form has been duly signed by the Accounting Staff.
• Ensure that all required signatories have signed your Clearance Form and that the signing follows the designated order indicated on the form before claiming your Test Permit/Admission Slip at the Accounting Office.
