Incoming Kindergarten:
1. Filled-out Modified Learner Enrolment and Survey Form ( Ihatag kini sa school nga libre)
2. Clear Photocopy of Birth Certificate (PSA)
3. Certificate of Completion gikan sa Day Care (Photocopy)

Incoming Grade 1
1. ECCD checklist
2. Filled-out Modified Learner Enrollment and Survey Form
3. Clear photocopy of Birth Certificate (PSA)
4. Photocopy of Certificate of completion (Kinder)

Incoming Grade 2-Grade 6
1. School Form 9 ( Report Card)
Filled-out Learner Enrollment and Survey Form
Clear Photocopy of Birth Certificate (PSA)
