HEALTH SERVICE UNIT PRE-ENROLLMENT GUIDE (JUNE 2026)
To ensure a smooth enrollment process, please review the updated Health Service Unit (HSU) medical requirements and step-by-step process below.

REQUIREMENTS CHECKLIST:
Find your category below and prepare your documents in one (1) long size ordinary folder:
For 1st Year, Transferees, TCP, & Returnees:
-Medical certificate
-Accomplished Student Health Record Form
-Chest X-ray Result (PA View)
-Complete Blood Count (CBC)
-Drug test result
-One (1) latest 2x2 picture (with name tag & white background)
-Photocopy of school ID and Notice of Admission
-Photocopy of PWD ID (if applicable)
For Continuing Students (2nd to 4th Year):
-Updated Medical Certificate
-Accomplished Student Health Record Form
-One (1) latest 2x2 picture (with name tag & white background)
-Photocopy of school ID
-Photocopy of PWD ID (if applicable)
-Other requirements as deemed necessary
STEP-BY-STEP PROCESS:
1. Download & Print Forms: Obtain the Student Health Record form from the link above and print it out. Fill out the required information completely.
2. Complete Medical Tests: Visit a licensed laboratory or clinic to get your laboratory tests done (Chest X-ray, CBC, and Drug Test for incoming/new students).
3. Medical Checkup & Interpretation: Bring your laboratory test results to the Rural Health Unit (RHU) or any accredited hospital for a medical checkup and the official interpretation/signing of your medical certificate.
4. Organize Documents: Secure all original forms, signed medical certificates, laboratory results, photocopies, and 2x2 photos inside a long folder.
5. Submit & Validate: Proceed to the Health Service Unit on your designated date for final review and enrollment clearance.

June 22: 4th Year
June 23: 3rd Year
June 24: 2nd Year
June 25 - 27: 1st Year (Freshmen)
June 26: TCP, Transferees, & Returnees
June 27: Late Enrollees
