Todayโs Ikigai and electives session showed us that becoming emergency physicians means aligning our passion, mission, vocation, and profession. As a group, we saw our elective choices not just as requirements, but as deliberate steps toward a meaningful and purpose-driven path. In that intersection, we begin to find our North Starโand who we are becoming. Together, we move forward with clearer direction and shared purpose.

A Clinical Simulation on Breaking the Bad News in the Emergency Department
Today's simulation on breaking bad news taught us that words carry weight, but silence carries something more. In the chaos of the emergency department, we often ramble like programmed machines, delivering news efficiently but emptily. We fill every pause with medical words because silence feels uncomfortable. But we learned that the most important moment after saying "He has died" is not the next sentence. It is the five seconds of nothing. That silence is not empty. It is where grief lands. Where tears finally fall. Where a family realizes they are not alone. We learned to stop talking. To just sit. To let time do what medicine cannot.
We commit to treating bad news like any other emergency procedure. We will prepare. We will pause. We will not leave the family after the news is over. Because no one should be alone after their world breaks. In the end, the most human thing we can give is not an explanation. They will not remember every word we said. But they will remember that we stayed. And sometimes, that is enough.
We would like to thank the Division of Supportive, Hospice and Palliative Medicine and the UPCM Simulation Laboratory. Your expertise and compassion made this simulation a truly transformative learning experience for all of us.

Gaps in Emergency Department triage systems highlight the need for action beyond the bedside, addressing workforce support, resource allocation, and health policy, while confronting social determinants of health.
As emergency medicine specialists, we must lead clinical innovation, institutional change, and policy advocacy to build safer, more equitable, and resilient emergency care systems.
Thank you, Dr. Baquiran, for an enlightening lecture on โBakit parating puno ang PGH ER?โ
