ANOTHER SCHEDULE OF PHYSICAL EXAMINATION OF QUALIFIED FRESHMEN APPLICANTS on AUGUST 7, 2023, Monday, 10:00AM-11:00AM only

1. Print UHS Form No. 8 as your laboratory request for chest x-ray, urinalysis, blood type and complete blood count. Have it done at any private/public hospital or DOH Accredited Diagnostic Clinic.

2. Print UHS Form No. 4 - Health Examination Record Form . Attach your latest passport size photo on the upper right portion of the form. On the front page answer the upper portion only and on the back page answer it completely. Accomplish the form at home.

3. Bring the Health Examination Record Form, results of CBC , urinalysis, blood type and chest x-ray on the date and time of scheduled physical examination at the PLM University Clinic.
