Application for the TESDA-Accredited Caregiving NC II Program in CGHC is now open.
This comprehensive program is designed to equip individuals with the necessary competencies in caregiving over a duration of six (6) months, encompassing a total of eight hundred and five (805) hours.
Caregiving applicants are required to submit the following enrollment requirements:
โขAccomplished Application Form
โขTwo (2) copies of the Student Information Sheet (Printable via the automated student account
โขOriginal and photocopy of the High School Report Card (Form 138) or Form 137 (for High School Graduates)
โขOriginal and photocopy of the Honorable Dismissal or Certificate of Transfer Credentials (for College Graduates and Undergraduates)
โขOriginal and photocopy of the Transcript of Records (TOR) (for College Graduates and Undergraduates)
โขOriginal and photocopy of the ALS Certificate and corresponding Grades (for Alternative Learning System passers)
โขOriginal and photocopy of the Certificate of Graduation (for High School and College Graduates)
โขOriginal and photocopy of the Certificate of Good Moral Character
โขOriginal and photocopy of the PSA-Authenticated Birth Certificate
โขOriginal and photocopy of the PSA Marriage Certificate (if applicable)
โขOriginal and photocopy of the Medical Clearance Form issued by the College Physician
โขOne (1) piece 2x2 and two (2) pieces 1x1 colored ID photographs
โขOne (1) long ordinary folder
โขOne (1) piece long plastic envelope
