• Students paying individually are required to submit a photocopy of their School ID.

• Class Presidents are required to submit a photocopy of their School ID together with a signed Waiver of Liability Form, printed on either letter-size or legal-size paper, upon payment collection.

• Freshman students are advised to prepare PHP 140.00 on their assigned schedule.

• Only cash payments will be accepted. Online payments are not allowed.
