Application/Admission Requirements for the Doctorate Degree:

1. Letter of Intent (addressed to the Dean, Dr. Guillermo C. Bungato)
2. Filled-up Recommendation for admission (to be accomplished by immediate supervisor/head of office)
3. Downloaded and signed PUP CBA GSEE Application Form
4. Photocopy Transcript of Records (from previous school attended)
5. Photocopy Birth Certificate (PSA Authenticated)
6. Photocopy Certificate of Marriage (for married female applicant only)
7. Certificate of Employment
8. Comprehensive Resume
9. One copy ID picture, 2"x2" (recent and in formal attire w/ white background)
