1. Income Return Tax (ITR)
2. Baranggay Certificate of Indigency
3. Any government-issued documents that indicate the income earning of the member(s) of the household who would be responsible for financing of the student's cost of education

The requirements shall be submitted at your respective Departments before June 15, 2017.

WHERE: At TUP Grounds

WHAT TO WEAR (COSTUME FOR MASS DANCE) : any type of POLO SHIRT its either short or long sleeves. And BLACK shirt or sando (printed shirt are allowed).

*NOTE: Before 6:00 or exact should be at TUP Grounds. If you came at 6:30 you will not allow to perform the Mass Dance because the mass dance performance will be start before the parade.
