👉🏻 The parents must bring the child upon enrollment for recording purposes of Height and Weight.

👉🏻 Prepare for Registration Fee.

1. 1pc PHOTOCOPY of BIRTH CERTIFICATE or LIVE BIRTH CERTIFICATE with REGISTRY NUMBER
2. 1pc PHOTOCOPY of child's IMMUNIZATION CARD
3. The child must be 4 years old
4. The parents should BRING their CHILDREN during REGISTRATION for RECORDING PURPOSES of HEIGHT and WEIGHT of the CHILD
5. Prepare for registration fee
6. Bring your own ballpen
