CED-LS: GRADE 7 - INFORMATION (SY 2026-2027) Please fill out the required details below. This form will serve as your confirmation of admission to the university as a Grade 7 student. Kindly answer the form honestly. The form will be open from May 21 to May 22, 2026.

We are pleased to announce the enrollment schedule for Grade 8, 9, and 10 learners. Kindly take note of your designated time slot and report accordingly.

Before anything else — if you haven't filled out the pre-registration form yet, do it now! Scan the QR code for your grade level and complete the form on or before May 18, 2026, 12:00 NN. Links will close after that.

📅 Here's when to come in:
Incoming Grade 8 — May 19, 2026
Incoming Grade 9 — May 20, 2026
Incoming Grade 10 — May 21, 2026

🕘 Enrollment runs from 9:00 AM to 2:00 PM on your assigned day, so make sure you come on time!

On enrollment day, here's what to keep in mind:
💳 Bring and surrender your school ID to the Lab School
💰 Come ready to pay your miscellaneous fees — these are settled on the day itself
👕 Type C uniform is allowed
👨‍👩‍👧 Feel free to bring a parent or guardian with you

Check if your name is on the list and proceed to OB Room 306 on May 13, 2026, 9:00 AM – 3:00 PM for enrollment. Please come with a parent or guardian — their presence will be needed for decisions during the process.

📌 If you have not yet submitted any of the following requirements — Form 138, GMC, or PSA Birth Certificate — please bring them on the day of enrollment for filing.

Place the following inside a long brown envelope and write your complete name at the back top corner:

✔️ Original copy of F138 (Report Card)
