To join the sports clinic and tryout evaluations, submit the following to the Sports Development Office before your tryout date:

𝟏. 𝐏𝐚𝐫𝐞𝐧𝐭𝐚𝐥 𝐂𝐨𝐧𝐬𝐞𝐧𝐭 𝐅𝐨𝐫𝐦 – Signed waiver from a parent or legal guardian
𝟐. 𝐌𝐞𝐝𝐢𝐜𝐚𝐥 𝐂𝐥𝐞𝐚𝐫𝐚𝐧𝐜𝐞 – Certification that you are physically fit to compete
𝟑. 𝐕𝐚𝐥𝐢𝐝 𝐈𝐃– School ID or birth certificate copy

𝐑𝐄𝐌𝐈𝐍𝐃𝐄𝐑𝐒 𝐎𝐍 𝐓𝐑𝐘𝐎𝐔𝐓 𝐃𝐀𝐘
