For students who received a grade of 5 in any subject, submission of the appropriate form is required as follows:
• Academic Warning Form – for first-time cases
• Academic Probation Form – for second-time cases and for students with more than one (1) grade of 5

In addition to the required form, students with a grade of 5 must also submit the following documents:
• A copy of their grades
• A Salaysay ng Pangako

Students who received grades of INC, 4, W, or DRP are not required to submit any academic remediation documents.

Please comply accordingly and submit the complete requirements within the given period.
