3. Photocopy of Certificate of Completion ECCD gikan sa Day Care Center.

B. Alang sa umaabot Grade I Learners

2. Filled-out Modified Learner Enrollment and Survey Form (Anaay form nga pwede mapangayo sa eskwelahan).
3. Photocopy of Birth Certificate (kun mahimo PSA copy) labi na sa mga TRANSFEREES.
4. Certificate of Completion (Kinder) labi na TRANSFEREES.

C. Alang sa mga TRANSFEREES (GRADES 2-6)
1. SF9- School Report Card
2. Filled-out Modified Learner Enrollment and Survey Form (Anaay form nga pwede mapangayo sa eskwelahan).).
3. Photocopy of Birth Certificate.
