Applicants must submit the following requirements:
1. Letter of Intent or Letter of Application addressed to the Head of Office;
2. Duly accomplished PDS (CS Form No. 212, Revised 2017) in two copies with the latest 2x2 pictures;
3. Certified photocopy of valid and updated PRC License/ ID;
4. Certified photocopy of Eligibility or Ratings obtained in the Licensure Examination for Teachers (LET)
5. Certified True copy of Transcript of Records (TOR) and Diploma
6. Photocopy of Certificate of Employment (COE), service record, performance rating, and school clearance for those with teaching experience from a private school
7. Certificate of Special Training, if any;
8. Certified copy of Voter's ID or any proof of residency;
9. NBI Clearance;
10. Checklist of Requirements and Omnibus Certification of Authenticity and Veracity of the documents submitted, signed by the applicants, and notarized by authorized officials.

Buong puso ang aming pasasalamat kina EPS ng Filipino, Dr. Jocelyn DR Canlas, EPS ng ESP/HG/Kindergarten, Dr. Cythia DG Yanga, at D7 PSDS, Sir Rommel C. Cruz para sa kanilang mahusay at mapusong monitoring and technical assistance.
