1. TDP APPLICATION FORM WITH 2x2, which can be downloaded from the link https://shorturl.at/G8H3J (print it in short-sized bond paper)
2. PHOTOCOPY of 2024-2025 iD with 3 signatures below the ID copy
3. CERTIFICATE OF INDIGENCY which can be requested from your respective barangay

Reminder: The submitted documents will be scanned and will be sent to UNIFAST. The UNIFAST will then decide if what will happen to those names whose documents will not reach unifast.

This prestigious scholarship is awarded to graduated Senior High School students last SY 2024-2025 who have consistently demonstrated outstanding academic performance, strong leadership potential, and a passion for learning.

✔️ Freshmen who have already validated their SIC Enrolment
✔️ Freshmen who have graduated in their SHS as an Academic Honor Award Awardee (A Highest Honor, High Honor, or Honor Awardee)

📍 Application Forms are available at the Registrar's Office. You can claim after you validate your SIC Enrolment.

✔️Validated Enrolment Form (for verification)
✔️Clear copy of SHS Report Card
✔️Original Certification the applicant graduated with Academic Excellence Award - With Signature of Principal/Registrar and with School Dry Seal)
Note: do not submit the actual Certificate received during the Awarding Ceremony
✔️Signed Declaration of Non-enjoyment of Government or Private Scholarship Programs (already attached to the application form)

SUBMIT THE NEEDED DOCUMENTS NOT LATER THAN JUNE 3, 2025. THE DOCUMENTS WILL BE SUBMITTED ON JUNE 4, 2025. THOSE WHO WILL NOT COMPLETE DOCUMENTS WILL BE DELISTED FROM THE TDP PROGRAM FOR FAILING TO SUBMITTED THE NEEDED DOCS REQUIRED BY UNIFAST.
