5. Commissioner (Guidance Counselor / Guidance Designate, or any teaching or non-teaching personnel who is a members of the school Child Protection Committee) Grievance Committee

▪️Batang Kindergarten completer.
▪️Original Kinder Report Card
▪️ Photocopy ng PSA Birth Certificate
