📌 Reminders for University ID Registration
For Officially Enrolled Students – A.Y. 2025–2026
All officially enrolled students are required to register for their University ID by completing the form and submitting the necessary requirements. Please read and follow the guidelines carefully:

✅ Step 1: Fill out the Google Form
• Access and complete the registration form using this link: https://forms.gle/REAjobyeQ1wZbFJM9
• Make sure to provide accurate and complete information.
• Upload all the required documents in the form.

✅ ID Picture Requirements:
• Must be a formal photo (with proper attire; no filters, no casual clothes, or accessories like hats/sunglasses).
• Taken by a professional studio or equivalent quality.
• Should be in softcopy (digital format) and must be the original digital file provided by the photo studio — not a printed studio photo that was merely photographed using a cellphone camera. Poor-quality or non-compliant photos will not be accepted.]
• Use a Plain white background only.
• Must be in JPEG format (.jpg or .jpeg) only.
• Selfies, group photos, or cropped photos from other images are strictly not accepted.
✅ Digital Signature Requirements:
• Sign on a clean white sheet of paper using black or blue ink.
• Scan or take a clear, high-quality photo of your signature.
• Save in JPEG or PNG format.
• Ensure the image is free of shadows, folds, or unnecessary marks.
✅ Latest COR:
• Upload a copy of your latest Certificate of Registration (COR) issued for A.Y. 2025–2026.
• File must be clear and readable, in PDF or JPEG format.
• Your COR must show that you are officially enrolled.
✅ File Naming Format:
Please use the following format for each file you upload:
• ID Photo: LastName_FirstName_IDNumber.jpg
• Signature: LastName_FirstName_Signature.jpg
• COR: LastName_FirstName_COR.jpg
(Example: Cruz_Juan_20253421.jpg, Cruz_Juan_Signature.jpg, Cruz_Juan_COR.jpg)
⚠️ IMPORTANT:
❗ Submissions that do not follow the guidelines (wrong file type, incorrect photo, missing documents, etc.) Or those that are incomplete or incorrect
👉 WILL NOT BE PROCESSED.
