Upon Enrollment, Please Bring the Following:
• 2 photocopies of NSO/PSA Live Birth Certificate
• 3 copies of 2×2 ID picture (white background)
• 1 copy of 1×1 ID picture (white background)
• 2 photocopies of Immunization Card (vaccination record only)
