Part of the University's preparation and compliance to requirements of the government for the gradual opening of face-to-face classes is ensuring that majority if not all of the members of the academic community are vaccinated. The University has its own vaccination program in coordination with the Department of Health and the Department of Information and communication Technology. However, not all personnel had their vaccination in the University vaccination center, some availed of the vaccination programs of their respective local government units (LGUs).

In order to come up with a complete data on the vaccination status of all personnel and students, you are hereby directed to urgently accomplish the simple survey from using the link: 𝗵𝘁𝘁𝗽𝘀://𝗳𝗼𝗿𝗺𝘀.𝗴𝗹𝗲/𝘂𝗵𝗔𝗛𝘅𝘀𝗵𝗕𝟵𝟰𝗪𝗽𝗖𝟲𝗬𝘅𝟵 which should be submitted 𝐨𝐧 𝐨𝐫 𝐛𝐞𝐟𝐨𝐫𝐞 𝟏𝟐:𝟎𝟎 𝐍𝐍 𝐭𝐨𝐦𝐨𝐫𝐫𝐨𝐰, 𝟏𝟒 𝐎𝐜𝐭𝐨𝐛𝐞𝐫 𝟐𝟎𝟐𝟏, 𝐓𝐡𝐮𝐫𝐬𝐝𝐚𝐲, to give time to the University vaccination team and the Office of the Vice President for Academic Affairs to consolidate and come up with the report which is also due for submission on the same day to the Senate Finance Sub-Committee through the Philippine Association of State Universities and Colleges (PASUC).

Further, considering the urgency of the matter, all Deans and Directors, Heads and Chiefs of Offices, USC and CSC Officers, BUUFAI and BUANTS officers are directed to disseminate widely this memorandum for immediate compliance of all concerned.
