- 1 printed copy of screen shot as evidence of having LBP account (A4)
📍Claim by

- 3 authenticated copies of school ID with 3 specimen signatures FRONT and contact number of the grantee (A4)
- 3 Original copies of SPECIAL POWER OF ATTORNEY stating the name of the parent, relationship (father/mother), name of grant (TDP/TES) with original signature
- 3 photo copies of any valid ID with 3 specimen signatures FRONT and contact number of the parent at the back (A4).
- 1 printed copy of screen shot as evidence of having LBP account (A4).
