TRANSCRIPT OF RECORDS (TOR) ISSUANCE

Please be informed that the Official Transcript of Records (TOR) will be available for claiming on February 3, 2026, 9:00 to 11:00 AM and 1:00 – 3:00PM, at the DFCAMCLP OCR– BA Campus, Topland Ave., Golden Gate Subd., Talon Tres, Las Piñas City.

IMPORTANT REMINDERS:
1. TOR will only be issued to those who have submitted their Admission Documents (F137/TOR from previous school) and the Student Clearance.
2. Bring school/valid ID and ballpen.

GUIDELINES:

1. Kindly take note of the schedule below:

Dec. 1, 2025 (Monday) - A to C
Dec. 2, 2025 (Tuesday) - D to J
Dec. 3 2025 (Wednesday) - L to N
Dec. 4, 2025 (Thursday) - P to Y

TIME: 9:00 to 11:00 AM
1:00 to 3:00 PM

2. The Transcript of Records (TOR) will be issued only to students who have completed and submitted the following requirements:

a. Admission Documents (F137/TOR from previous school)
b. Duly accomplished Student Clearance.

3. Please bring school/valid ID and ballpen.

Please be informed that your Special Order (S.O.) Number is still being processed by CHED-NCR. Your Transcript of Records (TOR) will be released once the S.O. Number has been issued.

In the meantime, you may request the following documents for employment, further studies, or other purposes:
1. Certificate of Grades
2. Certificate of Graduation
