Kindly submit the documentary requirements ON OR BEFORE SEPTEMBER 15, 2026 to the REGISTRAR'S OFFICE.

REQUIREMENTS TO SUBMIT:
Accomplished Tulong Dunong Application Form (Version 2024)
- Attach one (1) recent 2x2 ID picture
- Forms are available at the Registrar's Office
Certificate of Registration (COR) or Registration Form
- First Semester 2025-2026

Requirements for Personal Claim:

Two (2) valid IDs (original and photocopy) with three (3) specimen signatures below the photocopy (preferably School ID and PhilHealth ID or any other government-issued valid ID).

If an immediate family member will claim the check on behalf of the student-grantee, the following documents must be submitted:

Duly signed Authorization Letter from the student-grantee;

Original and photocopy of valid IDs of both the student-grantee and the authorized representative, each with three (3) specimen signatures;

Photocopy of the student-grantee's Landbank ATM Card, with three (3) specimen signatures beside the photocopy.

Important Reminder:
If a student-grantee authorizes a representative to claim the check on their behalf, the check will be issued as a crossed check and not be encashable. It must be deposited directly into the student-grantee's personal savings account under their name.

Attention to all TES Beneficiaries listed in the TES Masterlist (1st Semester, A.Y. 2025–2026).

All listed beneficiaries are advised to open a Land Bank of the Philippines (LBP) account and submit their account details through the provided Google Form by scanning the QR code or using the link indicated in the attached document.
