at 1st Street Purok Kapayas Inside Mactan Printing, Canjulao Lapu Lapu City
Monday to Thursday
8:00 AM to 3:00 PM

pls bring the following
- Photocopy of PSA or Live birth
- Photocopy of Immunization Card
- Ballpen
